The NCACC Board of Directors and its Executive Committee make policy decisions to guide the NCACC and its staff through the year. The board consists of:
- Five officers
- 18 district directors
- Elected members of the National Association of Counties (NACo) Board of Directors
- Up to five at-large commissioners
- Several ex-officio members who chair NCACC committees or hold other appointments
- Up to five Past Presidents of the NCACC who are still active county commissioners

President Johnnie Carswell from Burke County was sworn in as NCACC President at the 116th NCACC Annual Conference in 2023.

First Vice President Wallace Nelson from Perquimans County was elected at the 115th NCACC Annual Conference in 2022.

Immediate Past President
Past President Tracey Johnson of Washington County was sworn in as NCACC President at the 115th NCACC Annual Conference in Cabarrus County. She has served as Chair of the Board of Washington County Commissioners, Chair of the Albemarle Commission Council of Government, and on the Board of Directors of the National Association of Counties (NACo). She has also served on numerous boards and committees for topics including: mental health systems (Trillium Health Resources Board of Directors), educational systems (new schools and lottery advisory committees), risk management, recreation, aging, and crime stoppers, among others.

Kevin Leonard serves as the NCACC Chief Executive Officer of the association and oversees all activities of the association. He provides day-to-day administrative oversight and coordinates advocacy, research, communications, member services and internal support functions.

NCACC has 18 multi-county districts represented by directors elected to two-year terms, who also serve on the NCACC Board of Directors. The odd-numbered districts elect their representatives during the NCACC Annual Conference, which is held every August, in odd-numbered years. The even-numbered districts elect their representatives during the NCACC Annual Conference in even-numbered years.
