Description

The Community Alternatives Program for Disabled Adults (CAP/DA), CAP/DA/CHOICE/MFP is a Medicaid Program designed to enable the nursing home level of care to enable a client to return to or to remain in his/her home.

Provide social work services in the areas of adult/children services which involve the assessment of individual and family dynamics and needs.  Work involves independent screening assessments, develop plan of care which includes making referrals to meet financial, psychological, medical, emergency, or other care needs along with obtaining home mobility aids (HMA), durable medical equipment (DME), waivers supplies, etc.  Decisions of the SWII’s directly impact the client and families socially, psychologically, emotionally and physically.  SWII may explain diagnosis and treatment alternatives to client and families along with counseling. Knowledge of medical terminology and disease processes, mental retardation/developmental programs and behavior management techniques are necessary to maintain clients in the home.

Home Visits-assess and develop a care plan meeting the clients’ needs as determined through the understanding and utilization of social work assessment techniques and treatment approaches.  Directly related experience is defined as human services experience in the areas of case management, assessment and referral, supportive counseling, intervention, psycho-social therapy and treatment planning.  A comprehensive knowledge of available services is required.

Salary

Starting $17.05+ depending on experience

Qualifications

Master’s degree in social work from an appropriately accredited institution and one year of directly related experience; or a bachelor’s degree in social work from an appropriately accredited institution and two years of directly related experience; or a master’s degree in a human services field from an appropriately accredited institution and two years of directly related experience; or a bachelor’s degree in a human services field from an appropriately accredited institution and three years of directly related experience; or a bachelor’s degree from an appropriately accredited institution and four years of directly related experience; or an equivalent combination of education and experience.
